Jeff Davis Parish Live Oak Chapter, Daughters of the American Revolution seeking information on members of the United States Military -- Veterans honored in publication
The Live Oak Chapter of the Daughters of the American Revolution is proud to announce the beginning process of preparing volume two of the Pictorial Directory of Jefferson Davis Parish Louisiana Patriots and Veterans.
The women in the Live Oak Chapter cherish all the men and women who were willing to sacrifice their lives in protecting American citizens. This book will be honoring citizens who fought in any conflict or war. Therefore citizens who lived in, or is living in Jeff Davis Parish and have served in the military any time (past and/or present) are encouraged to fill out the Record of Service form to share their stories. Citizens of this parish who have relatives serving, or have served in the military (living and/or deceased), are also encouraged to fill out the Record of Service form for those relatives. Please call Mary Sue Lyon at (337) 734-3811 for the Record of Service form. The simple form consists of questions such as veteran’s full name, parents, mother’s maiden name, veteran’s rank, place of induction, Unit(s), dates served, war(s), medals/honors, battles fought, military training such as schools or special training, birth place and resident of, (death date, burial place and cemetery), marriage date, place, spouses’ maiden name, children and their residences and phone numbers, brothers and sisters and their residences and phone numbers, and a place for a signature of consent.
The completed forms, photos and a photocopy of DD214 Discharge of the Veteran are to be sent before December 31, 2011 to Live Oak Chapter, DAR, Attention: Mary Sue Lyon, DAR Veteran Chairperson, 201 Pine Street, Welsh, Louisiana 70591. If sending information on up to two veterans that were/are in the service, the Live Oak Chapter DAR asks that one book be purchased at the pre ordered price of $30. If information on more than three veterans is sent in, Live Oak Chapter DAR needs an additional $2 per person (even if purchasing only one book). Live Oak Chapter DAR will return any photos if requested. We appreciate that keeping photos of family members is important.
The Live Oak Chapter, DAR has published 2009 Pictorial Directory of Jefferson Davis Parish Louisiana Patriots and Veterans, Volume One, telling stories of 304 veterans who served in the Army, Navy, Air Force, Marines, Merchant Marines, National Guard, Coast Guard, Reserves, Women’s Army Air corps, Galvez Expedition, Colonial, Revolution, Indiana Union, Virginia/West Virginia Militia, Confederates and Army Medical Corps.
